Description of Applicable New York Insurance Law
New York State Insurance Law § 2612 and related regulations establish provisions for persons insured by an individual insurance policy issued to another person (the “policyholder”) or a group insurance policy. Under this law, when an insurance company is provided with a valid Order of Protection against a policyholder or another person covered under the insurance policy, the insurance company is prohibited from disclosing the address and telephone number of the insured or of any person or entity providing services to the insured, to the policyholder or another person covered by the insurance policy without the express written consent of the individual who has made the request in accordance with these protocols. If a child is the insured, a parent or legal guardian may assert this right on behalf of the child.
In addition, an insurer may not discriminate against a person solely because he or she has been a victim of domestic violence.
To Initiate or Revoke a Request for Confidentiality
To initiate a request, the victim, the victim’s legal representative, or if the victim is a child, the child’s parent or guardian, shall deliver a valid court Order of Protection to the following mailing address or email address:

In addition to the valid Order of Protection, your submission (whether by U.S. Mail or email) must include an alternative address, telephone number or other method of contact. We ask that you also include your insurance policy number as part of your submission. Please be advised that CSAA Insurance Group has up to three business days to implement the necessary protocols to secure your personal information.
You may revoke this confidentiality request at any time by mailing a notarized letter of instruction to the address provided above.
